(1) This lead form remains active for one (1) year from the date submitted. Additional lead forms submitted for the same service within the one year will be rejected.

(2) Should a referral result in a sale, commission will be paid only for the named service(s) above.

(3) Any lead form requesting under 6 hours of additional consulting time will be rejected.

(4) Any lead form with more than one employee name will be rejected.
